Midland Forms is a subsidiary company of Midland Trust, a custodial service provider focusing on IRA accounts and private fund custody. Midland Forms is one of 3 subsidiaries under its parent company and offers document preparation services.

Because of this, Midland's employees are extremely knowledgeable across many disciplines and can answer an array of questions you might have.

We are employee-owned

In December 2019, Midland became an employee-owned company or an ESOP. This means all employees at Midland own stock in the company.

Being an employee-owned company benefits clients and employees alike because everyone succeeds when the company does. It fosters a culture built on responsive and impactful work each day.

Our history

Midland Forms began offering services in 2015 under the name IRA Doc Prep. It started as an extension to Midland IRA, servicing IRA clients. Midland Forms mainly focused on entity formation and incorporation services.

Since then, Midland Forms has had a name change and expanded its services to non-IRA clients and increased its services available. Starting in 2023, Midland Forms will offer Simple Wills and Land Trusts, as well as Note Extensions.
